Quillivant XR (methylphenidate) is a long-acting central nervous system (CNS) stimulant formulated to manage ADHD symptoms. In 2012, it was approved by the U.S. Food and Drug Administration (FDA) for the treatment of ADHD in adults and children 6 and up.
In clinical trials, Quillivant XR was found effective for reducing the symptoms of ADHD, including impulsivity, lack of focus, and hyperactivity. It’s available as brand-name Quillivant XR, which is an extended-release (XR) liquid formula. There’s also an extended-release chewable tablet called QuilliChew ER (methylphenidate).
Quillivant XR and QuilliChew ER are controlled substances that can be abused or cause dependence.
Quillivant XR and QuilliChew work on the central nervous system by increasing dopamine and norepinephrine levels in the brain. Dopamine and norepinephrine are neurotransmitters (chemical messengers) that carry signals in the brain.
Increased dopamine and norepinephrine levels improve communication between brain cells, which helps focus, attention, and impulse control. By boosting these neurotransmitters, Quillivant XR can improve concentration and reduce hyperactive and impulsive behaviors.
Quillivant XR provides a steady dose of medication for 12 hours to help manage symptoms throughout the day and into the evening. And Quillivant XR begins working quickly. Its effects are felt as soon as 45 minutes after taking it.
QuilliChew ER is a chewable extended-release tablet that lasts up to 8 hours. It begins working rapidly and reaches peak efficacy around 5 hours after a tablet is taken.
Quillivant XR and QuilliChew ER are FDA-approved for children 6 and older and adults who have ADHD.

Quillivant XR is a liquid, extended-release suspension. QuilliChew ER is a chewable tablet. Quillivant XR comes in a bottle at a strength of 25 milligrams per 5 milliliters. The bottle includes a dispenser for dosing.
QuilliChew ER chewable tablets come in 20-, 30-, and 40-milligram strengths.
The recommended starting dosage for both Quillivant XR and QuilliChew ER is 20 milligrams daily, taken in the morning. The maximum dose of either is 60 milligrams a day. If you need more than 20 milligrams a day, your provider can adjust your dose of either form gradually in increments of 10 milligrams to 20 milligrams a week.
You can take Quillivant XR and QuilliChew ER with or without food.
If you need to stop taking Quillivant, your provider will have you decrease your dose gradually to reduce the chance of withdrawal symptoms.
If you have side effects from Quillivant or QuilliChew, the most common ones include decreased appetite, trouble sleeping, nausea, vomiting, indigestion, stomach pain, weight loss, anxiety, dizziness, irritability, mood swings, fast heart rate, and increased blood pressure. These side effects are often mild and may decrease as you continue taking Quillivant or QuilliChew.
Less common but more serious side effects can include increased heart rate, psychotic or manic symptoms, and possible growth delays in children. Quillivant XR and QuilliChew ER may not be recommended to patients with heart abnormalities or cardiac disease because deaths have occurred in patients who take CNS stimulants.
Because they’re controlled substances, Quillivant and QuilliChew come with an FDA-boxed warning for potential abuse and misuse, which can lead to substance abuse.
Other warnings include but aren’t limited to, the potential to cause serious heart issues, increased blood pressure and heart rate, worsening of pre-existing psychosis, mania, vascular problems, harm to patients with phenylketonuria (PKU), and even sudden death.
According to drugs.com, with its discount, the cost of Quillivant XR oral suspension is around $366 for a 60-milliliter bottle. QuilliChew ER tablets cost around $1,317 for a supply of 100 20-, 30-, or 40-milligram tablets.

Adderall and Quillivant have different active ingredients, but Quillivant and Ritalin have the same. Adderall contains amphetamine/dextroamphetamine compared to Quillivant and Ritalin’s methylphenidate.

What sets Quillivant and QuilliChew apart from non-methylphenidate medications and other methylphenidate medications are their formulations. Quillivant is also the only long-acting liquid form of methylphenidate. While other forms of methylphenidate are available as dissolvable tablets, only QuilliChew is a chewable tablet. You can though crush standard Ritalin tablets and add them to a small amount of soft food as an alternative to QuilliChew.
The Quillivant XR liquid suspension doses can be tailored more precisely than stimulants in tablet form. Both Quillivant XR liquid suspension and QuilliChew ER are alternatives for individuals who can’t swallow pills. Adderall and Ritalin, especially their generic versions, are often more affordable than Quillivant. There’s currently no generic for Quillivant or QuilliChew.

Two common non-stimulant options are norepinephrine reuptake inhibitors (NRIs) Strattera (atomoxetine) and Qelbree (viloxazine).
