  • Insurance Eligibility & Verification Data: If you provide insurance information, we may collect information to assist providers in verifying eligibility.

  • Device Information: IP address, browser type, operating system, and referral sources.
  • Cookies & Tracking Technologies: We use cookies and similar technologies to improve website performance and user experience.

  • To facilitate provider payments and self-pay transactions.
  • To store and transmit insurance-related data to providers for eligibility verification.
  • To assist providers in confirming insurance benefits through third-party verification services.

Klarity does not submit claims or determine insurance coverage decisions. Providers are responsible for all claims processing and reimbursement matters.

  • With Providers: To facilitate appointment scheduling, payment collection, and insurance verification.
  • With Third-Party Service Providers: We may partner with benefit verification, billing, and claims submission vendors. These vendors operate under Business Associate Agreements (BAAs) where required by HIPAA.
  • With Insurers (Only by Providers): Klarity does not submit PHI or claims to insurers. Providers handle all insurance-related PHI disclosures.

A. Accessing & Controlling Your Information

  • Request a copy of the insurance-related data Klarity collects.

  • Klarity does not process insurance claims or determine coverage.

Encryption & Security: All stored insurance-related data is protected under AWS encryption standards and HIPAA-compliant security policies.

Data Retention: Insurance-related data is retained according to our standard record retention policies. Klarity does not impose extended retention periods due to payer requirements.

Non-PHI Handling: If insurance-related data does not qualify as PHI, it is handled under our general data policies and may not be subject to encryption.

7. Third-Party Integrations

Klarity integrates with third-party services for benefit verification and administrative support. These third parties have their own privacy policies governing their handling of patient information.
