What is oral semaglutide for weight loss?

Oral semaglutide, branded as Rybelsus, is a glucagon-like peptide-1 (GLP-1) receptor agonist. Its active ingredient, semaglutide, is also available in the injectable semaglutide medications, Ozempic and Wegovy. Oral semaglutide may be preferred to injectable formulations for people who are uncomfortable with needles and interested in taking GLP-1s for weight loss.

The U.S. Food and Drug Administration (FDA) approved oral semaglutide under the brand name Rybelsus to treat type 2 diabetes, in combination with diet and exercise, in 2017. While oral semaglutide is currently only approved for diabetes management, healthcare providers can prescribe it off-label for weight loss, thanks to its appetite-suppressing effects.

The drug’s manufacturer, Novo Nordisk, is currently seeking approval for oral semaglutide for weight loss. In clinical trials, people taking oral semaglutide lost 15.1% of their body weight in 68 weeks, while those taking a placebo lost 2.4%.

There’s no generic of Rybelsus, but semaglutide is available as a compounded medication if your healthcare provider determines that’s the right treatment for you.

How does oral semaglutide weight loss work?

Oral semaglutide mimics the action of the GLP-1 hormone, which is naturally produced in the gut. The medication works for weight loss by:

  • Slowing the rate at which food leaves your stomach, helping you feel full for longer.
  • Curbing hunger by sending satiety (“fullness”) signals to your brain, so you eat fewer calories.
  • Reducing your cravings for certain foods, especially sugary and fatty foods, so you have more control over what you eat.

In a clinical trial comparing oral semaglutide to injectable liraglutide (Victoza), people with type 2 diabetes lost more weight on oral semaglutide in 26 weeks (half a year). The average weight loss for those taking oral semaglutide was 9.7 pounds, compared with 6.8 pounds for those taking Victoza and 1.1 pounds for those taking a placebo.

That clinical trial used the lower doses of oral semaglutide currently available with Rybelsus. The drug’s manufacturer is seeking approval for oral semaglutide for weight loss using higher doses, based on the results of a recent clinical trial. In this trial, people taking 50 milligrams of oral semaglutide lost 15.1% of their body weight in 68 weeks (about a year and a half). If your initial body weight is 250 pounds, that’s a weight loss of 37.5 pounds.

Who can take oral semaglutide for weight loss?

Oral semaglutide is FDA-approved for adults with type 2 diabetes who need help lowering their blood sugar levels. If you take it, you should adopt a healthy diet and exercise more.

It’s essential to note that while weight loss is an off-label use, medical professionals consider oral semaglutide a viable option for many patients with obesity or overweight.   • Swallow the tablet whole with a small amount of water (4 ounces).
  • Wait at least 30 minutes before eating, drinking, or taking other medications.

Oral semaglutide side effects and warnings

While oral semaglutide is generally well-tolerated, it can cause side effects. The most common side effects include:

  • Nausea
  • Vomiting
  • Diarrhea
  • Abdominal pain
  • Constipation

These symptoms are more common at the higher 14-milligram dose and often subside with time as your body adjusts to the medication. Serious but rare side effects are also possible with oral semaglutide, including an allergic reaction to the medication, pulmonary aspiration during surgery, and kidney, gallbladder, or pancreas problems. Among people with type 2 diabetes, oral semaglutide can increase the risk of diabetic retinopathy and hypoglycemia (low blood glucose) when combined with insulin or other glucose-lowering medications.

What does oral semaglutide cost?

The cost of oral semaglutide varies depending on your insurance coverage and pharmacy. Currently, there’s no generic version of oral semaglutide. There’s only brand-name Rybelsus. For a 1-month supply, the average out-of-pocket price for Rybelsus is $1,029, according to drugs.com and with its discount. Compounded versions of oral semaglutide may be more affordable, but you’ll want to make sure you purchase it from a reputable, licensed pharmacy.

Several websites provide up-to-date pricing for oral semaglutide with pharmacy-specific discount cards you can use to lower the cost of your prescription. Sites include GoodRx, WellRx, SingleCare, Optum Perks, NeedyMeds, and drugs.com.

Note that if you take oral semaglutide for weight loss, it’s less likely to be covered by insurance because weight loss is an off-label use. If your insurance does cover oral semaglutide, they’ll likely require prior authorization.

How does oral semaglutide compare to other weight loss medications?

Compared to other weight loss medications, oral semaglutide is less effective at its currently available dosage strengths. Clinical trials show that people taking the maximum 14-milligram dose of Rybelsus lost an average of 9.7 pounds over 26 weeks.

In comparison, weight loss pills such as Contrave (bupropion-naltrexone) and Qsymia (phentermine-topiramate) can result in 5 to 10% weight loss over 56 weeks. For someone with an initial body weight of 250 pounds, that translates to 12.5 to 25 pounds in about a year.

Weekly injections, including Wegovy, which contains the same active ingredient as oral semaglutide, have produced even greater weight loss results. In a 68-week clinical trial, people taking Wegovy lost an average of 14.9% of their initial body weight in 68 weeks. In a 72-week trial of Zepbound (tirzepatide), another injectable GLP-1 receptor agonist, people lost up to 20.9% of their initial body weight.

These medications are FDA-approved for weight reduction or weight management in individuals with obesity or overweight, when used alongside a reduced-calorie diet and increased physical activity. Common side effects vary depending on the drug but may include nausea, vomiting, diarrhea, constipation, and headaches. They can also cause serious side effects.
