What is naltrexone?

Naltrexone is an opioid antagonist medication. In 1984, it was approved by the U.S. Food and Drug Administration (FDA) for the management of opioid addiction. In 1994, it was approved for the treatment of alcohol use disorder (AUD). A secondary benefit of naltrexone is that it can lead to weight loss. Though it’s not FDA-approved for weight loss, it may be prescribed off-label to support weight reduction.


Since the 1990s, providers have used low-dose naltrexone (LDN) off-label to help treat neurological conditions and address inflammation. For weight loss, it may help reduce appetite. LDN is typically taken in doses ranging from 0.5 to 5 milligrams and may be available at compounding pharmacies.

Naltrexone is available under the generic name naltrexone hydrochloride. It’s also the active ingredient in oral tablets sold as the brand names ReVia and Depade. Naltrexone can also be taken as a long-acting injection under the brand name Vivitrol.


Contrave (naltrexone HCI/bupropion HCI) combines naltrexone and Wellbutrin (bupropion). It’s FDA-approved to treat obesity. Bupropion is an antidepressant medication that can reduce appetite. Naltrexone can reduce food cravings. Together, they can lead to decreased appetite and feeling fuller.

How does naltrexone for weight loss work?

Naltrexone affects the brain’s reward system and works by blocking the euphoric effects of alcohol and opioids. It can lead to weight loss by reducing food cravings and food consumption. A study examining naltrexone and reward-based behavior found that a 50-milligram dose reduced the reward eating drive (RED) and the intensity of food cravings for sugary and fatty foods in women with obesity.


Combining naltrexone with a medication like bupropion, which reduces appetite, can lead to more significant weight loss. Research suggests that within the first 12 weeks of starting Contrave (naltrexone-bupropion), a person with obesity can lose about 5% of their body weight.

A decreased appetite can lead to less food intake and less weight gain. Blocking cravings for foods high in sugar and fat can make it easier to choose more nutritious foods. Naltrexone for weight loss dosages and how to use it

Naltrexone comes in 2 forms: oral tablet and injection. The generic, naltrexone hydrochloride, and the brand name versions, ReVia and Depade, are available as 50-milligram tablets.

Your provider may recommend starting with 50 milligrams of naltrexone for weight loss every morning for the first week. As your body adjusts to the medication, your provider may add an evening dose and continue to increase your morning and evening doses as medically necessary.

If you and your provider decide that LDN is a good option for you, they may start you on a dose of 0.5 milligrams and gradually increase your dosage to up to 5 milligrams.

Vivitrol is a brand-name injection that contains 380 milligrams of naltrexone. Once every 4 weeks, a healthcare provider administers this intramuscular gluteal (buttocks) injection.

Contrave is a fixed-dose medication that combines naltrexone and bupropion. Each extended-release tablet combines 32 milligrams of sustained-release naltrexone and 360 milligrams of sustained-release bupropion. Learn more about Contrave.

Naltrexone side effects and warnings

Like other medications, there’s a risk of side effects when taking naltrexone. The most common side effects of oral naltrexone for weight loss (generic, ReVia, and Depade) are nausea, headache, dizziness, nervousness, fatigue, insomnia, vomiting, anxiety, drowsiness, abdominal cramps, increased thirst, bone and joint pain, cold-like symptoms, and restlessness.


Naltrexone is contraindicated (not recommended for use) when someone is currently dependent on opioids or opioid medication like methadone or buprenorphine, people taking opioid analgesics, or anyone in active opioid withdrawal because there’s a potential risk of overdose. Your provider won’t recommend naltrexone if you have a history of sensitivity to the medication.


If you’re considering naltrexone, it’s important to know that it has warnings. In some cases, naltrexone can lead to hepatotoxicity or liver damage. When studied for the treatment of opioid dependence, depression, suicidal thoughts, attempts, and death by suicide were reported. Your provider will monitor your progress on the medication and make adjustments to your dose or care plan if problems occur.


People taking Vivitrol may experience injection-site reactions like tenderness, swelling, and itching. Other side effects of Vivitrol are similar to oral naltrexone.


Common side effects for Contrave are similar as well.

What does naltrexone cost?

The cost of generic naltrexone, according to drugs.com and with its discount, is $48 for 30 50-milligram oral tablets. It lists the cost of 30 50-milligram ReVia tablets as $118. The cost of LDN will depend on the specific compounding pharmacy you use. Because doses are smaller, using LDN may help you save money.

Vivitrol, the long-lasting naltrexone injection, is significantly more expensive. One 380-milligram injection can cost $1,737.59 with the drugs.com discount.

Extended-release Contrave oral tablets have 8 milligrams of naltrexone and 90 milligrams of bupropion. A supply of 120 oral tablets can cost $667.84.

How does Naltrexone compare to other weight loss medications?

Research shows that naltrexone can lead to some weight loss. In a systematic review, people taking naltrexone lost an average of 1.7% of their body weight after 24 weeks. Other weight loss medications may lead to greater weight loss over time.

Zepbound (tirzepatide) and Wegovy (semaglutide) are FDA-approved for weight loss in people with obesity or overweight when used alongside changes in diet and physical activity. Clinical trial data shows that people taking Zepbound lost 20.9% of their body weight in 72 weeks, while those taking Wegovy lost an average of 14.9% of their body weight in 68 weeks.
