In clinical trials, people taking 4.5 milligrams of dulaglutide — the active ingredient in Trulicity — for type 2 diabetes lost an average of 10.1 pounds in the first 9 months. If you weigh 200 pounds, that’s 5% of your body weight!

Dulaglutide is the active ingredient in the brand-name medication Trulicity, which is made by Eli Lilly. In September 2014, Trulicity was approved by the U.S. Food and Drug Administration (FDA) for the treatment of type 2 diabetes. There’s no generic of dulaglutide available yet.
Dulaglutide in Trulicity isn’t FDA-approved for weight loss. But, some people taking dulaglutide have lost weight. For that reason, some providers prescribe dulaglutide off-label (for something it’s not FDA-approved for) to support weight management.
Dulaglutide, the active ingredient in Trulicity, is a glucagon-like peptide-1 (GLP-1) agonist medication. It mimics GLP-1, a natural hormone that helps regulate eating. The medication reduces appetite by signaling your brain that you’re full. Dulaglutide also slows the movement of food through your digestive tract and improves how your body uses insulin to help regulate blood sugar levels. Higher blood sugar levels can make you feel hungry.
Eating nutritious reduced-calorie meals, increasing movement, and taking dulaglutide can help with blood sugar control, manage appetite, and lead to weight loss.
In clinical trials, people taking 4.5 milligrams of dulaglutide as the active ingredient in Trulicity once a week lost an average of 11.1 pounds in 1 year.
Dulaglutide as the active ingredient in Trulicity is FDA-approved to improve blood sugar (blood glucose) control in people 10 and older who have type 2 diabetes mellitus when used with diet and exercise. It’s also approved to reduce the risk of major cardiovascular events, like heart attack, stroke, or death, in people with heart disease or cardiovascular risk factors. Weight loss may be a secondary benefit of taking dulaglutide for diabetes care, but using it only for weight loss is not an FDA-approved use.
Dulaglutide helps your body better use insulin and can lower your A1C (a test used to measure average blood sugar levels). In adults with type 2 diabetes mellitus, dulaglutide also reduces the risk of serious cardiovascular events like cardiovascular death, nonfatal myocardial infarction, or nonfatal stroke in people with a history of cardiovascular disease or risk factors.
Some providers prescribe dulaglutide off-label to support weight loss. But, they consider many factors like your health status, weight, and if you have any existing medical conditions. Dulaglutide has benefits, but there are also risks. So, a provider will talk with you to consider dulaglutide along with other options before prescribing it, especially if you don’t have type 2 diabetes.
To get a dulaglutide prescription online, start by talking to a trained, licensed healthcare provider. A qualified provider can assess your overall health and needs to determine if dulaglutide or an alternative option is medically necessary for you.
Qualified providers include physician assistants/associates (PAs), nurse practitioners (NPs), and medical doctors (MDs). You can find a skilled, board-certified provider on Klarity Health in as little as 24 hours, share your concerns and goals, and discuss a weight management plan that may include a medication like dulaglutide.*
If your provider decides that dulaglutide is a medically necessary part of your treatment plan, they can send a prescription to the pharmacy of your choice.
Dulaglutide is a once-a-week injection. Each dose comes in a prefilled, single-use pen. On the same day each week, you use the pen to inject the medication under the skin in your belly, thigh, or in the back of your upper arm.
Doses of Trulicity are available in 0.75-, 1.5-, 3.0-, and 4.5-milligram strengths.
The starting dose is typically 0.75 milligrams once a week for 4 weeks. Starting with a smaller dose gives your body time to adjust to the medication and helps reduce side effects. After a month, you and your provider will have an idea of how you’re tolerating the medication. If you’re tolerating it well, your provider may increase your dosage to the next higher dose every 4 weeks until you reach your maximum or maintenance dose, which may be up to 4.5 milligrams a week.
Clinical studies show that people lose more weight with higher doses of dulaglutide. After 9 months, people taking 1.5 milligrams lost an average of 6.6 pounds, while people taking 4.5 milligrams lost 10.1 pounds. If you struggle to tolerate higher dosages, your provider may keep you on a lower dose.
If you take dulaglutide in Trulicity or as a compounded medication and miss a dose, take the missed dose as soon as possible unless your next dose is due more than 3 days. If your next dose is due in less than 3 days, skip the missed dose and simply take your next scheduled dose at the normal time.
There are benefits and risks with taking once-weekly dulaglutide in Trulicity. In addition to weight loss, Trulicity can help manage blood sugar levels and appetite. Having better control over blood sugar levels can reduce the risk of long-term complications from diabetes. Trulicity can also reduce the risk of serious cardiovascular events, like heart attack or stroke, in people with type 2 diabetes.
People taking it can experience side effects. Common side effects are gastrointestinal and include nausea, diarrhea, vomiting, abdominal pain, decreased appetite, and indigestion. Fatigue is a common side effect too. When starting Trulicity, nausea occurs most often but typically subsides after the first 2 weeks. People may also experience more nausea when increasing their dose.
Dulagludie in Trulicity can cause serious side effects like lower blood sugar, pancreatitis, or gallstones (cholelithiasis). It can also cause adverse reactions like kidney failure or severe stomach problems, and interact with other drugs, like insulin. It comes with an FDA-boxed warning for the possibility of thyroid C-cell tumors like medullary thyroid carcinoma (MTC) and multiple endocrine neoplasia syndrome type 2 (MEN 2).
If you take Trulicity as dulaglutide and have side effects, work with your provider and consider reporting any negative side effects to the FDA by visiting MedWatch or calling 1-800-FDA-1088.

Trulicity is a GLP-1 receptor agonist like Ozempic and Wegovy. The active ingredient in Trulicity is dulaglutide, while semaglutide is the active ingredient in injectable medications Ozempic and Wegovy. Trulicity and Ozempic are FDA-approved for the treatment of type 2 diabetes mellitus. Wegovy is approved for weight loss and management in people with a high body mass index (BMI). All 3 are approved to reduce the risk of serious cardiovascular events. When used for diabetes care and coupled with increased physical activity and a reduced-calorie diet, Trulicity, Ozempic, and Wegovy can help with weight reduction.
Clinical trials show that at high and low doses people lose more weight on Wegovy and Ozempic than Trulicity. In clinical trials, people taking 2 milligrams of Ozempic for diabetes lost 5 to 10% of their body weight in 40 weeks. With changes to diet and exercise, people taking Wegovy saw an average weight loss of 14.9% in 68 weeks. Clinical trial participants lost between 3% and 5% of their body weight with Trulicity in 52 weeks.
Without insurance, the cost of all doses of Ozempic on drugs.com with its discount is $1,029 for a 1-month supply, while Trulicity costs $1,039. Wegovy is the most expensive, costing $1,430 for a 1-month supply of any dose without insurance.
Trulicity, Ozempic, and Wegovy can all lead to weight loss when combined with nutritious reduced-calorie meals and increased movement. All have side effects that may include nausea, diarrhea, vomiting, abdominal pain, decreased appetite, and fatigue. Ozempic and Wegovy have the same FDA-boxed warning for the risk of thyroid C-cell turmors as Trulicity.
